Physicians Orders for Life Sustaining Treatment (POLST) is an effective, evidence-based program that is rapidly being implemented across the United States to improve respect for patient wishes. It began in Oregon in the early 1990s as a program to help assure that patient's health care preferences surrounding treatments near the end of life were honored. POLST is a set of medical orders based on signed by a physician (or in some states a nurse practitioner or physician assistant) based on a conversation with the patient or surrogate. These medical orders are then used by EMS, emergency departments, long-term care facilities and other health care professionals in the event of a crisis. POLST Paradigm Programs have now been established and endorsed by the National POLST Task Force in over 13 states and POLST is receiving increasing national attention. This presentation will provide a history or the development of POLST and the current national status of POLST Paradigm programs, along with practical information on how chaplains can help assure that health care professionals and patients have meaningful conversations about patient's preferences and how POLST can help assure those preferences are honored.

Terri Schmidt MD MA is a professor of Emergency Medicine and Palliative Care, as well as a senior scholar, Center for Ethics in Health Care at Oregon Health & Science University, Portland, OR. She is the EMS Medical Director of Clackamas County, Oregon. She is one of the original developers of the POLST program and has been an invited speaker on the topic throughout the U.S. and internationally.
