FORMS TO REQUEST CHANGE OR RENEWAL OF STATUS The following forms allow board certified and associate chaplains to change their status to retired status or inactive status, or request renewal of inactive status. Retired Status Request Form  (Word Document) Download this form if you are currently a board certified or associate chaplain in good standing and who is retired from professional chaplaincy. Retired status applies to those who have substantially retired from the profession of chaplaincy and who work no more than 50% of the time within a year in professional compensated chaplaincy. Inactive Status Request Form  (Word Document) Download this form if you are currently a board certified or associate chaplain in good standing that is no longer active in professional chaplaincy and would like to request inactive status.
